If you have not done so listen to all lectures BEFORE attempting to complete the observation. Your grade will be based on providing evidence you watched AND applied the ethics of documentation.

Observation #1 – Self Help Skills (Observation Practice / Anecdotal Record 25points)

You will watch the video titled: Henry at Mealtime below. While watching the video you will complete an anecdotal observation on Henry in the area of self-help skills.

Anecdotal/Running Record Form

In the Observing Section: “What actually happened/What I saw:” section, type your anecdotal notes (just the facts, what you saw and heard). Children’s quotes would be appropriate here. Remember this is where your OBJECTIVE notes go. Only the facts!
